It was developed in Australia as a very simple test for psychological distress. To take the test, please fill in a value for the following field(s) which best describes how each statement applies to you. The questions refer to how you have felt and behaved over the last 4 weeks.
1 = None of the time. 2 = A little of the time. 3 = Some of the time. 4 = Most of the time. 5 = All of the time. The higher your score, the greater the psychological distress you will be experiencing due to depression or anxiety.
